Challenge Coin Request Form

The Shively Fire Department is selling challenge coins. The coins are $10 per coin and can be purchased by contacting the officer in charge at (502)-449-5025 extension 2 or filling out the form below.

The coins must be picked up in person at the Shively Fire Department and paid for with cash.
